The Allergy and Asthma Clinic of Northeast Georgia accepts new patients of all ages. Prior to your
first visit it is important that you read all new patient information and complete the pre-admission forms found at the link below.

Bring all information with you to your appointment.
Here are a few important things to remember
when
visiting our office for the first time:
1) Refrain from taking any antihistamines for at least THREE (3) days prior to your appointment. If you have any questions
regarding whether or not certain types of medications are antihistamines please contact your pharmacist, the prescribing
physician or a member of our staff.
If you are unable to discontinue using antihistamines, please keep your appointment and let a member of our nursing staff
know that you are currently taking an antihistamine.
CONTINUE TAKING ALL OTHER FORMS OF MEDICATION
THAT HAVE BEEN PRESCRIBED FOR YOU !
2) Bring a list of all medications that you are currently taking for any condition including
vitamins, supplements or over–the–counter drugs.
3) Bring the name, address and phone number of your primary care physician to facilitate continuity of care.
4) Give at least 24 hours advance notice if you are unable to keep your appointment or you will be charged
for an office visit.
5) Bring a current copy of your insurance card. If your insurance policy requires a referral number from your primary care
physician please make sure that you obtain this information prior to your appointment.
6) Refrain from wearing perfumes or colognes to our office as certain odors can cause allergy/asthma attacks in other
patients. This request includes persons who accompany patients to their appointment.
